What transition support may include
Housing Transition Support is hands-on housing navigation. It typically builds on the Personalized Housing Action Plan from your Housing Stability Assessment and focuses on the priorities in that plan.

Assessment clients who upgrade within 7 calendar days receive a $200 credit, leaving $1,050 due.
Additional professional support beyond the hours included in your service package is available at $95/hour with prior client authorization.

Simon Resources provides nonclinical housing consulting and navigation services throughout San Diego County. Virtual services are available, and eligible paid services may also be provided in person at the client's home or another agreed location.
Housing providers and assistance programs make their own eligibility and approval decisions.

We help you understand and evaluate housing options, prepare applications and coordinate next steps. We do not sell access to rental listings, negotiate lease terms, or guarantee housing placement or approval. Clients make their own housing decisions and agree to rental terms directly with housing providers.
With your authorization, we can support communication with housing providers about applications, documents and scheduling.
